About the Role:
The Case Manager plays a pivotal role in coordinating comprehensive care plans for patients within the health care services sector, ensuring that individuals receive timely and appropriate support tailored to their unique needs. This position requires close collaboration with multidisciplinary teams, including medical professionals, social workers, and community resources, to facilitate seamless service delivery and optimize patient outcomes. The Case Manager will assess patient needs, develop individualized care plans, monitor progress, and adjust interventions as necessary to promote health and well-being. Additionally, the role involves advocating for patients, navigating complex healthcare systems, and ensuring compliance with regulatory standards and organizational policies. Ultimately, the Case Manager serves as a critical liaison between patients, families, and providers to enhance quality of care and support holistic health management.
Minimum Qualifications:
- Bachelor’s degree in social work or human service-related field and minimum of one year experience and/or training with human or community services, or any equivalent combination of education and experience.
- Strong knowledge of healthcare systems, community resources, and patient advocacy principles.
- Excellent communication and interpersonal skills to effectively interact with patients, families, and multidisciplinary teams.
- Proficiency in maintaining detailed and confidential patient records in compliance with HIPAA and other regulatory standards.
Responsibilities:
- Collaborate with the patients, families, RN Care Managers, providers, and members of the healthcare team to address the social and psychosocial needs that directly impact the patient’s health outcomes.
- Assists with the SDOH screening and outreach process, data entry into the EHR, and evaluation of outcomes.
- Has an in-depth knowledge of the various human services agencies and support groups/services available in the community. Acts as a resource for patients, families, and the Enabling Services team.
- Helps patients and families navigate through the human service and medical systems.
- Provides support and resources to patients and families seeking additional care services or transitions from home to other facilities.
- Assists with end-of-life discussions and provides education and support to patients and families.
- Collaborates with the RN Care Managers in the care planning process to identify and evaluate patient centered goals, interventions, and outcomes.
- Assists with the identification of and planning for impactful outreach opportunities. Attending community events to promote access to medical care and community resources.
- Collaborates with the Community Based Case Manager to provide coverage and assistance as needed at the CASD Engagement Center.
- Utilizes reporting measures to track patient follow up and identify outcomes of care that is provided.
- Participates in available training and networking opportunities to develop and expand knowledge and understanding of available community resources.
- Promote and reinforce patient centered medical home concepts with patients and staff.
- Participate in patient centered care management meetings, interdisciplinary team meetings, and quality improvement initiatives.
