Keystone Health Sliding Fee Discount Program
The policy of Keystone Health is to provide patients who qualify with an opportunity to participate in a program reducing the amount owed to Keystone Health for services received. The major determining factors in this program are household income and size. You must be a current patient in order to qualify.
Once you are approved for the Sliding Fee Discount Program, you will receive a participant card indicating your level of reduction and a cover letter explaining where the card is honored. You must present your card at the time of each visit.
