Your New Patient appointment is used to establish your care with your new provider.
Your provider would prefer not to do a physical or a form completion during this visit, and would like to spend this time getting to know you and your medical history
During your 1st appointment:
1. Your provider will gather information and familiarize himself/herself with your past health history.
2. You may discuss future goals and/or testing that may be needed.
In order to avoid delays in treatment or medication refills, it is extremely important to do one of the following:
1. Have your past medical records/shot records sent to us prior to this appointment.
OR
2. Bring your past medical records / shot records with you when you arrive for this appointment.
Please understand that a follow up appointment may be necessary in order to address any additional concerns with your provider.
