Triage RN Nurse

We are a fast-paced, expanding Community Health Center looking for a Triage Registered Nurse to join our team and help improve the wellness of our neighbors. Keystone Internal Medicine treats patients that are 18 years of age and older.

Previous office nursing experience and knowledge of electronic medical record is preferred. This nursing position completes many tasks such as answering phone calls, lab/ diagnosis tracking, request medical records for new patients, referral tracking, and completes authorization for medications as needed. Applicant must also be willing to assist other nurses when needed. The applicant must have excellent critical thinking skills, be able to multitask, be well organized, and be able to work in a fast-paced environment. This position also requires the applicant to work independently and at times, in a team setting.

Spanish-speaking skills are a plus.

Keystone Agricultural Worker Program (AWP) Southeast Region is seeking a full-time Bilingual Case Manager to join the team.

The Bilingual Case Manager is responsible for assessing and addressing the social determinants of health (SDOH) for migrant and seasonal farmworkers. This role also involves providing health education to migrant and seasonal farmworkers at farmworker homes, farms, labor camps, and other outreach settings.

Duties include screening, interviewing, and assessing farm workers for SDOH disparities. Appropriately refers and enrolls patients to these services. Arrange follow up for patients for provider-placed consultation referrals. Assist patients with the completion of hospital-based charity care programs applications and enrollment. Collaborates with outreach coordinators at monthly outreach coordinator meetings. During clinic hours, covers front desk operations including greeting patients, check-in, privacy notices, and payment collection. Attends weekly regional staff meetings. As available, accompanies multidisciplinary teams on outreach trips to farmworker housing. Acts as an in-the-field social worker as SDOH questions arise. Assists with registration and screening of farmworkers as needed. Provides non-judgmental,

linguistically, and culturally appropriate health education to migrant and seasonal farmworkers. This education includes but is not limited to the following topics: Chronic disease prevention through diet and exercise, diabetes and hypertension, alcohol and drug abuse, mental health and depression, prevention and recognition of sexually transmitted infection, general recommendations for age-appropriate immunizations, and cancer screening tests. Registers farmworkers eligible to participate in the Keystone Agricultural Worker Program according to Program guidelines. Assists the regional coordinator in identifying farms employing migrant and seasonal farmworkers. Offers counseling services as licensure permits.

Qualifications: Bachelor’s Degree in Social Work strongly preferred or degree in relevant field with commensurate experience in social work.

Language Skills Requirement: Must be able to read, write, and speak English and Spanish fluently.

Keystone Health is seeking a full-time Bilingual Clinical Nurse Manager to join the team.

This position is located in Gardners, PA.

Oversees the clinical portion of the office and clinical staff (RNs, MAs, and LPNs) for the Statewide Agricultural Worker Program, while assuring quality patient care in accordance with Keystone Health’s (KH) mission. Works in collaboration with the Medical Director (Clinical), the Agricultural Worker Program Director (Administrative), and the Outreach Coordinators to ensure operational success of outreach activities and clinic-based activities.  Serves as a role model in attitude, attendance, behavior, professional appearance, communication, and interactions with other employees and patients.

Qualifications: Graduate of an accredited RN program.

Prefer 2 years of experience in Primary Care and/or Community Health work. 

Knowledge of and the ability to provide services in a manner appropriate to the culture of the farmworker community.

Licenses and Certifications: Current Pennsylvania RN nursing license. Current CPR certification.  Current PA Driver’s license. 

Ability to read, write and speak English and Spanish proficiently is required.
