Emergencies When the Office Is Closed – If you have a serious emergency, immediately call 911 or go to the hospital emergency room. When the office is closed, if you have a serious dental problem that is not an emergency but can’t wait until we reopen, call 709-7940 and follow the directions on our automated system.
Appointments – Please call 709-7940 to make appointments. Whenever possible we will make the appointment with your chosen provider. However, there may be times when he or she is not available at the time you would like your appointment. In that case, you may choose to schedule an appointment with another provider or wait until your provider of choice is available.
Cancellation of Appointments – If you can’t keep your appointment, please call 709-7940. A 24-hour notice is required so that we can give another patient that time.
Dental Records – The information in your record is confidential and will not be given out without your written permission. If you want your records given out, please come in to sign a release and provide us with a mailing address to which your records will be forwarded or click here to download a release form. There may be a charge for this service.
Return of Telephone Calls – Because our providers have a full-schedule of patients to see each day, they may not be able to return your call until the end of the day.
Payment for Services – Services are available to everyone. We participate with MetLife, Delta Dental, United Concordia and accept Medical Assistance. We have a sliding fee scale for those who qualify, and we will work with patients to set up a payment plan if necessary. Your payment is due at the time of service unless other arrangements have been made.
Billing Questions – Call 709-7940 during regular office hours if you have a question about your bill.
Complaints, Suggestions, or Comments – We welcome your concerns, suggestions, and comments so we can continue to make our services better. Please talk to any of our staff or ask to speak to a manager if you have a concern or comment. If your concerns remain unanswered, you have the right to notify the Joint Commission on Accreditation of Healthcare Organizations at www.jcaho.org or by calling 1-800-994-6610.
