Keystone Health is dedicated to providing exceptional patient care through the highest standards of pharmaceutical services. We are seeking a skilled and compassionate Clinical Pharmacist to join our team.
Key Responsibilities:
- Pharmaceutical Care: Provide pharmaceuticals and drug information as needed for adequate patient care.
- Offer counseling to patients on the use of prescription and over the counter medications and advise physicians about medication therapy.
- Review and interpret physician prescriptions; enter medication orders into patient profiles and verify the accuracy of patient profiles concerning drug incompatibility, interactions, allergies, and sensitivities. Consult with prescribers to clarify or change medication orders.
- Advise patients, physicians, and other health practitioners on the selection, dosages, interactions, and side effects of medications. Monitor the health and progress of patients to ensure safe and effective medication use.
- Medication Assessment: Assess the identity, strength, and purity of medications. Correctly select, prepare, dispense, and label the appropriate dosage form. Proficiency in compounding is required.
- Maintain records such as pharmacy files, patient profiles, charge system files, inventories, control records for radioactive nuclei, and registries of poisons, narcotics, and controlled drugs.
- Controlled Substances: Audit, dispense, and maintain correct records for controlled substances.
- Reporting and Compliance: Participate in reporting adverse drug reactions (ADRs), drug usage interventions, medication errors, incident reports, discrepancies, and consultations as outlined in policies and procedures.
- Administer immunizations as required.
Qualifications:
- Graduate of an accredited college or school of pharmacy with 2 (two) years of actual pharmacy experience.
- Valid Pennsylvania Pharmacist license
- Strong knowledge of pharmaceuticals, drug interactions, and compounding.
- Must have a history of a good driving record, and current PA driver’s license.
- Current CPR certification.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with healthcare professionals.
- Attention to detail and strong organizational skills.
- A passion for patient wellness and enjoy providing patient education.
