Office Hours – Monday through Friday 7:00 am – 5:00 pm.
Emergencies When the Office Is Closed – If you have a serious emergency, immediately call 911 or go to the hospital emergency room. When the office is closed, if you have a medical problem that is not an emergency but can’t wait until we reopen, call 387-8060 and our answering service will have a triage nurse call you.
Appointments – Call 387-8060 to make appointments. Whenever possible we will make the appointment with your chosen provider. However, there may be times when he or she is not available at the time you would like your appointment. In that case, you will be scheduled with another excellent provider. Please arrive 15 minutes before your scheduled appointment to allow time for check-in. We always do our best to be on time and appreciate your understanding when emergencies happen that could put us a little behind schedule.
Cancellation of Appointments – If you can’t keep your appointment, please call 387-8060 to let us know. We ask that you cancel at least 24 hours before your appointment, if possible, so that we can give another patient that time.
Prescription Refills – Please call 387-8060 a few days before your prescription runs out. We need at least 2 business days to get your prescription ready. Please note that prescriptions cannot be refilled during the weekend or after regular office hours.
Lab and Test Results – At your appointment, your nurse or provider will tell you how to get your lab results.
Medical Records – The information in your record is confidential and will not be given out without your written permission. If you want your records given out, please come to the office or mail your request telling us where the records should be sent. There may be a charge for this service.
Return of Telephone Calls – Someone from our office will return your call by the end of the business day.
Payment for Services – Services are available to everyone. We participate with most insurances and accept Medical Assistance and Medicare. We have a sliding fee scale for those who qualify, and we will work with patients to set up a payment plan if necessary. Your payment is due at the time of service unless other arrangements have been made.
Billing Questions – Call our Billing Office at 709-7922 Monday through Friday from 8:00 am to 5:00 pm if you have a question about your bill.
Complaints, Suggestions, or Comments – We welcome your concerns, suggestions, and comments so we can continue to make our services better. You can fill out a patient feedback form found in our office. If your concerns remain unanswered, you have the right to notify the Joint Commission on Accreditation of Healthcare Organizations at www.jcaho.org or by calling 1-800-994-6610.
