Dear Patient,
It has been a great pleasure providing for your dental health care needs over the years. I am writing to inform you of my retirement effective July 15, 2026.
Moving forward, Dr. Alexandra Sehne will become your new dental provider, and my partnering dental hygienist, Carrie Derk, will continue to support your dental care needs.
You do not have to do anything except make your next appointment when you are ready. Dr. Sehne will have access to all of your dental records.
If you choose a provider outside of Keystone, a copy of your confidential dental health records can be transferred to your new provider or released to you or another person you designate (only with your permission).
I have greatly valued our relationship and thank you for your loyalty over the past years. Best wishes for your future health.
