You do not have to be a patient of Keystone Health to receive the COVID vaccine at any of our clinics.
We offer pediatric COVID booster vaccines at multiple locations. To schedule your child’s COVID booster vaccine, please call your preferred office- locations and phone numbers listed below. We are able to give COVID vaccines at Well Child Visits, or you can schedule an appointment specifically for a COVID vaccine.

If you are able, please print and fill out the consent form (English- click here) (Spanish, click here) and bring with you to the appointment. Otherwise, it can be filled out at the appointment.
Cost:
COVID-19 vaccine is provided at 100% no cost to the vaccine recipient. If a vaccine recipient has health coverage, Keystone Health may seek appropriate reimbursement from the recipient’s plan or program for a vaccine administration fee.

Pfizer- First Dose (Pediatrics)

We offer pediatric COVID vaccines at multiple locations. To schedule your child’s COVID vaccine, please call your preferred office- locations and phone numbers listed below. We are able to give COVID vaccines at Well Child Visits, or you can schedule an appointment specifically for a COVID vaccine.
When scheduling an appointment, remember:
- Children age 5 years- 17 years: This is a 2 dose series. 2nd vaccine will be automatically scheduled exactly 21 days (3 weeks from 1st dose) for you. Please make sure you are available in 3 weeks for 2nd dose.
- Children age 6 months-4 years: This is a 3 dose series. 2nd vaccine will be automatically scheduled exactly 21 days (3 weeks from 1st dose) for you, and 3rd dose will be scheduled for you 8 weeks after the 2nd dose.
Please make sure you are available for follow up doses. Parent or guardian must accompany minors to vaccine appointments.

Pfizer Vaccine Now Available to Children 5+!
CDC recommends that all children ages 5 and up get vaccinated against COVID-19. Vaccination is over 90% effective at helping prevent COVID-19 in kids and is an important tool to help stop this pandemic.

COVID Vaccine Outreach Events
Hello! Thank you for partnering with us on our COVID-19 Vaccine outreach event. Please follow the below instructions to get your participants scheduled for vaccines.
What you need to do?
An appointment is required to receive a vaccine. Please use the scheduling tool below. If you do not have insurance information or social security number at the time of scheduling, please just use “zeros” in place of these numbers. The vaccine recipient should bring this with them at the time of their appointment.

**Note: after scheduling, you will receive an email with incorrect clinic location. The vaccine clinic will be at your business location as discussed with our outreach clinic coordinator.
Healthcare Worker COVID-19 Booster
This COVID-19 booster vaccine scheduling page should be used for healthcare workers that have received both doses of the Moderna or Pfizer vaccine at least 6 months ago, and wish to receive a Moderna booster dose.
Please schedule online, using the scheduling tool below. NOTE: Our online scheduling tool automatically asks for insurance information. As a healthcare worker, you will NOT be charged an administration fee.
Please bring signed and completed consent form (CLICK HERE for consent form), your ID and your insurance card so we can scan them when you arrive.

The vaccine clinic is located at the Keystone Professional Center located at 111 Chambers Hill Drive. Please proceed to second floor, and follow signs for the “Conference Center”.
