RN Care Manager
Family Practice
Full-Time, Monday – Friday
No Weekends, Holidays, or On-Call Hours
Keystone Health is seeking a full-time RN Care Manager to join the team.
The RN Care Manager would be responsible for collaborating with the interdisciplinary care team to identify patients with complex medical needs and to develop a patient-centered plan of care. The RN Care Manager will provide care coordination for high-risk patients that are transitioning from the hospital and other settings. The RN Care Manager will collaborate with the Enabling Services team and community resources to address barriers to care and provide individualized education to patients and families.
Qualifications: Associate degree in nursing required, Bachelor’s degree preferred. Minimum of three years working in a clinical setting as an RN. Experience in Care Management desired. Current CPR certification.
Requires strong interpersonal skills to communicate effectively with patients and families, both in person and virtually, and to collaborate with the care team members.
Must be able to demonstrate independent judgement and strong critical thinking skills.
