Provides Care Management services to patients by developing, monitoring and evaluating interdisciplinary care. The RN Care Manager behaves in a professional manner and consistently demonstrates and promotes the values of respect, honesty, care and dignity for the patient and all members of the healthcare team. The RN Care Manager will be committed to the constant pursuit of excellence in improving the health status of our patients and our community and will collaborate with the healthcare team, specialists, and patients/families to ensure the delivery of quality, efficient and cost-effective healthcare services.
Associate degree in Nursing required; Bachelor’s degree preferred. Minimum of three (3) years working in a clinical setting as RN required. Experience in care management desired. Current CPR certification.
