This position will be responsible for providing an increased focus on the social and psychological needs of Keystone’s patients by identifying social determinants of health, providing an in-depth array of resources, and making referrals to the care community.
Requires a Bachelor’s degree in Social Work or Human Services related field. Minimum of 1-year experience in a clinical or community setting preferred. Candidate must have strong interpersonal skills and an interest in working closely with patients, team members, and community organizations with diverse cultural and literacy levels.
